Application Submission

  1. Please download the application form here.

  2. Application procedure:

    a)
    The completed application form must be submitted to The School of Professional Education and Executive Development (SPEED) of The Hong Kong Polytechnic University at QR802, 8/F, Core R, PolyU, Hung Hom, Kowloon, Hong Kong, BY HAND during the following office hour OR BY POST to the above address.

    Monday – Friday 8:45a.m. – 12:45p.m.
      2:00p.m. – 5:15 p.m.
    Saturday 9:00 a.m. – 12:00 noon
    (excluding any Hong Kong Public Holiday)
       
    b)
    The deadline for application submission is 5:15 p.m. on 8 March 2007. Late entries will not be accepted.
       
    c)
    The organiser will not be held responsible for any loss/delay of application sent in by post.

  3. The application form must be properly filled in and signed with all required information submitted, otherwise it may not be considered.

  4. The application should also include one copy of each of the applicant's student identity card.

  5. The application is made on the understanding that the organiser will not be liable to pay any cost arising out of its preparation and submission.
